Investigating the impact of the COVID-19 pandemic on recovery colleges: multi-site qualitative study

Background
During the COVID-19 pandemic, mental health problems increased as access to mental health services reduced. Recovery colleges are recovery-focused adult education initiatives delivered by people with professional and lived mental health expertise. Designed to be collaborative and inclusive, they were uniquely positioned to support people experiencing mental health problems during the pandemic. There is limited research exploring the lasting impacts of the pandemic on recovery college operation and delivery to students.
AimsTo ascertain how the COVID-19 pandemic changed recovery college operation in England.
MethodWe coproduced a qualitative interview study of recovery college managers across the UK. Academics and co-researchers with lived mental health experience collaborated on conducting interviews and analysing data, using a collaborative thematic framework analysis.
ResultsThirty-one managers participated. Five themes were identified: complex organisational relationships, changed ways of working, navigating the rapid transition to digital delivery, responding to isolation and changes to accessibility. Two key pandemic-related changes to recovery college operation were highlighted: their use as accessible services that relieve pressure on mental health services through hybrid face-to-face and digital course delivery, and the development of digitally delivered courses for individuals with mental health needs.
ConclusionsThe pandemic either led to or accelerated developments in recovery college operation, leading to a positioning of recovery colleges as a preventative service with wider accessibility to people with mental health problems, people under the care of forensic mental health services and mental healthcare staff. These benefits are strengthened by relationships with partner organisations and autonomy from statutory healthcare infrastructures.

Clay minerals are abundant in soils and sediments and often contain Fe. Some varieties, such as nontronites, contain as much as 40 wt.% Fe2O3 within their molecular structure. Several studies have shown that various Fe-reducing micro-organisms can use ferric iron in Fe-bearing clay minerals as their terminal electron acceptor, thereby reducing it to ferrous iron. Laboratory experiments have also demonstrated that chemically or bacterially reduced clays can promote the reductive degradation of various organics, including chlorinated pesticides and nitroaromatics. Therefore, Fe-bearing clays may play a crucial role in the natural attenuation of various redox-sensitive contaminants in soils and sediments. Although the organochlorinated pesticide p,p′-DDT is one of the most abundant and recalcitrant sources of contamination in many parts of the world, the impact of reduced Fe-bearing clays on its degradation has never been documented. The purpose of the present study was to evaluate the extent of degradation of p,p′-DDT during the bacterial reduction of Fe(III) in an Fe-rich clay. Microcosm experiments were conducted under anaerobic conditions using nontronite (sample NAu-2) spiked with p,p′-DDT and the metal-reducing bacteria Shewanella oneidensis MR-1. Similar experiments were conducted using a sand sample to better ascertain the true impact of the clay vs. the bacteria on the degradation of DDT. Samples were analyzed for DDT and degradation products after 0, 3, and 6 weeks of incubation at 30°C. Results revealed a progressive decrease in p,p′-DDT and increase in p,p′-DDD concentrations in the clay experiments compared to sand and abiotic controls, indicating that Fe-bearing clays may substantially contribute toward the reductive degradation of DDT in soils and sediments. These new findings further demonstrate the impact that clay materials can have on the natural attenuation of pollutants in natural and artificial systems and open new avenues for the passive treatment of contaminated land.

Life course research embraces the complexity of health and disease development, tackling the extensive interactions between genetics and environment. This interdisciplinary blueprint, or theoretical framework, offers a structure for research ideas and specifies relationships between related factors. Traditionally, methodological approaches attempt to reduce the complexity of these dynamic interactions and decompose health into component parts, ignoring the complex reciprocal interaction of factors that shape health over time. New methods that match the epistemological foundation of the life course framework are needed to fully explore adaptive, multilevel, and reciprocal interactions between individuals and their environment. The focus of this article is to (1) delineate the differences between lifespan and life course research, (2) articulate the importance of complex systems science as a methodological framework in the life course research toolbox to guide our research questions, (3) raise key questions that can be asked within the clinical and translational science domain utilizing this framework, and (4) provide recommendations for life course research implementation, charting the way forward. Recent advances in computational analytics, computer science, and data collection could be used to approximate, measure, and analyze the intertwining and dynamic nature of genetic and environmental factors involved in health development.

Summary
Introduction
Nutrition plays a key role in later life health and wellbeing. Older people face a high risk of nutrient deficiencies and malnutrition that can lead to sarcopenia, loss of skeletal muscle mass and strength. A recent review identified that sarcopenia was associated with functional decline, higher rate of falls, higher incidence of hospitalisations and increased mortality (Beaudart et al, 2017). Understandably severe sarcopenia is extremely disabling as it prevents independent living and places an increasing burden on care providers.
Avoiding late life malnutrition is dependent on a number of factors including physical, mental and cognitive health. However, the relative impact of each of these factors and the relationships between them are not well understood. Physical factors, such as problems with chewing, swallowing and impaired mobility, all contribute towards nutritional decline (Hickson, 2006). Mental health status also plays a part, particularly depression, which has been identified as a predictor of poor appetite in older adults (see Engel et al, 2011). Treating depression can be an effective way of increasing appetite and improving nutritional status, but it is commonly under-diagnosed and under-treated among older people (Allan et al, 2014).
There is also growing evidence of associations between diet and cognitive function. Older people with dementia or cognitive decline have a poorer nutritional status than those without (Atti et al, 2008), with increasing dementia severity related to poorer nutritional status (Riccio et al, 2007). The potential for diet to protect against cognitive decline in older people is not currently well understood as much of the epidemiological research has not been supported in trials, and more research is needed to confirm the impact of changing whole diets on cognitive measures (Smith and Blumenthal, 2016).
To explore the associations and interactions between mental and physical health and diet, new intervention and prospective cohort studies are needed (Psaltopoulou et al, 2008). Such research is challenging to complete with older adults, as both ageing itself and the accompanying cognitive and physical decline are progressive and dynamic. Existing tools for measuring diet, cognition and physical activity typically provide snapshots of the situation and cannot identify rate of decline nor readily distinguish cause and effect.

The amount of Arctic sea ice predicted by the Hadley Centre Global Cilimate Model (GCM) is evaluated using 15 years of passive-microwave data. While the Hadley model reproduces the seasonal cycle reasonably well, it underestimates the total area of sea ice by more than 3 × 106 km2 for most of the year. In the winter months, most of the underestimate in ice area results from the prediction of far too little ice in Hudson Bay and the Sea of Okhotsk, leading to an excess of up to 0.2 PW heat input to the atmosphere from Hudson Bay alone. The surface-energy budget of Hudson Bay is investigated using a mixture of surface observations (POLES), satellite data (ATSR, SSM/I and ISCCP) and output from the Goddard Data Assimilation Office analysis. Flux adjustments of the order of 200 Wm−2, resulting from anomalously high sea-surface temperatures in the Levitus (1982) climatology, are found to be the cause of the model’s underestimation of sea ice in both Hudson Bay and the Sea of Okhotsk. The fact that flux adjustments based on an inaccurate climatology will produce errors, even if the model physics is correct, underlines the need both for improved climatologies and for models accurate enough not to require flux adjustment.

Withdrawal of antipsychotic drugs from adults with intellectual disabilities

Objectives: There is a view that antipsychotic drugs can be successfully withdrawn from people with intellectual disabilities with resultant health gain. This study critically examines whether antipsychotic drug withdrawal is beneficial.
Methods: 119 adults with intellectual disabilities were included in a programme of antipsychotic drug withdrawal.
Results: The clinical outcomes of this programme are poor. Only 7.6% completely withdrew from antipsychotic drugs, and 48.7% experienced onset/deterioration in problem behaviours or mental ill-health. Significant drug side effects with the introduction of new drugs were experienced by 23.3%. Resultant mean antipsychotic drug doses were higher compared with those at the start of the programme. The cost to the intellectual disabilities psychiatric service (over and above that of routine psychiatric care) was £258,050 (€391,693), and the human cost was considered high. Primary care, social care and family costs were not calculated. The only specific factors found to be associated with poor outcome were increased severity of intellectual disabilities and female gender.
Conclusions: The successful role of antipsychotic drugs in the management of problem behaviours in people who do not have verbal communication skills may relate to the pathoplastic effect of intellectual disabilities on clinical presentation, with problem behaviours being markers of eg. psychosis and anxiety disorders. Our original findings challenge the view that routine withdrawal of antipsychotic drugs is good practice; we reinterpret the existing literature, and recommend caution and vigilance in pharmacotherapy.
